DO NOT FILE OR SUBMIT PAYMENT.<\/strong>\u00a0 We contacted the Kentucky Secretary of State\u2019s office last year regarding this form and this was their response\u2026\n<blockquote><span style=\"color: #808080;\">&#8220;It did not come from the Kentucky Secretary of State&#8217;s Office.
